Service Review Team (SRT)

The Service Review (SRT) team brings youth services, social services, school personnel, families, natural supports, and community providers to assist children and families struggling with behavioral or emotional issues. Youth are invited to join the SRT meeting if it is developmentally appropriate for them to do so because SRT members value their input. Cromwell Youth Services will also ask any identified strong supports to join the meeting at the family's request (coaches, mentors, outside-of-school therapists, close family members, etc., may be invited). 

During the meeting, the referring party and family share their perspectives of the presenting issues and root causes. SRT core members will prompt further explanation through dialogue and questions to make the best recommendations for youth and families. At the end of the SRT meeting, families leave with a focused action plan, resources, and support to help them gain access to the help they need. Anyone can make referrals; families themselves, school personnel, community agencies, etc.

An informational brochure is available below. The referring party will complete a referral form, and families are asked to sign a permission slip and release information to participate in SRT. If you have any questions about the SRT process or would like more information about how to make a referral, please contact Youth Services at 860-632-3448.
